This HIPAA Notice explains how protected health information may be used and disclosed and how clients may access their information. This page is a general template and should be reviewed by a qualified compliance professional or attorney before use.

Our Responsibility

Aligned & Accountable Community Care is committed to protecting the privacy and security of health information in accordance with applicable federal and state privacy laws, including HIPAA when applicable.

Use and Disclosure of Information

Protected health information may be used for treatment, care coordination, payment, healthcare operations, required reporting, legal compliance, and other purposes allowed or required by law.

Client Rights

Clients may have the right to request access to records, request corrections, request restrictions, request confidential communication, receive an accounting of certain disclosures, and receive a copy of this notice.

Authorizations

Written authorization may be required before sharing information with certain family members, agencies, schools, courts, or community partners unless disclosure is otherwise allowed or required by law.

Safeguards

We use reasonable administrative, physical, and technical safeguards to protect health information. Staff and contractors are expected to follow confidentiality and privacy procedures.

Complaints

Clients may submit privacy concerns to Aligned & Accountable Community Care at info@aandacommunity.com. Filing a complaint will not affect access to services.

Do not use website forms, email, or SMS for emergencies. Call 911 or go to the nearest emergency room.
