Aligned & Accountable Community Care may use SMS messaging to communicate with individuals who have requested information, services, appointment reminders, case coordination, or follow-up communication.

Program Description

SMS messages may include appointment reminders, intake follow-up, service coordination, referral updates, document reminders, and general administrative communication.

Opt-In

You may opt in by providing your mobile number through our intake process, contact form, referral process, or direct communication with our team.

Opt-Out Instructions

Reply STOP at any time to stop receiving SMS messages. Reply HELP for assistance.

Carriers

Mobile carriers are not liable for delayed or undelivered messages.

Eligibility

You must be the account holder or have permission from the account holder to receive SMS messages at the number provided.

Confidentiality Reminder

Standard SMS messaging may not be fully secure. Do not send urgent, emergency, or highly sensitive information by text.
